The disclosure is lawfully permitted or required (e.g., subpoena, investigation of fraudulent activity, request by regulator, etc.)

Vendors who receive the information in the above-mentioned circumstances, will be required to maintain a similar privacy policy to that of Columbia Bank.

Access to nonpublic personal information is restricted to only those bank employees who need to know the information in order to provide products or services to you. Physical, electronic, and procedural safeguards are in place to ensure the protection of your nonpublic personal information.

The bank and its staff are committed to ensuring the accuracy of our customer�s personal information. If you find that the information in our records is inaccurate, you may write to our Auditor at P. O. Box 1609, Lake City, FL 32056. We will research the inaccuracy and provide you with a written response.
